GENERAL POWER OF ATTORNEY
(Made in accordance with the Substitute Decisions Act, 1992)
I,[Name of person making the Power of Attorney] revoke any previous continuing power of attorney for property made by me and now hereby appoint [Name of the Attorney] to be my lawful attorney for property and other powers as defined under this Agreement.
If the person I have appointed above cannot or will not be my attorney because of refusal, resignation, death, mental incapacity, or removal by the court, the in such circumstances, I substitute [Name of Substitute] to act as my attorney for property with the same authority as the person he or she is replacing.
